Sports Medicine Fellowship Application Requirements
Successful completion of a residency in Family Medicine or Internal Medicine from an ACGME accredited program. To apply, the following materials are required:
- Completed AMSSM Application for Sports Medicine Fellowship: http://www.amssm.org/Fellowships.html
- Letter of intent describing your future plans in Sports Medicine
- Current Curriculum Vitae (CV)
- Three (3) letters of recommendation including one (1) from your Residency Program Director
- Dean's Letter
- Intent to take either the Family Medicine or Internal Medicine ACGME board exam
- Be licensed in NYS or licensure eligible in NYS
